This certificate confirms that | have been consulted by my doctor and remain unable to work due to medical reasons. The current estimated return to work date is June 15, 2026. This documentation substantiates my continued absence as being fully supported by medical facts related to my workplace injury. As previously stated, due to the active WorkSafeBC Prohibited Action Investigation (File 2026D316), all further communications from the Company must be sent via email or registered mail to maintain a clear administrative record.
