MH-

| filed a safety concern on Thursday regarding the noise level from the server located
near workstation #3

Friday- began having headaches and ear pain, more on the left side
Monday- went to ER regarding these issues, doctor found no issues related to ears

Safety tested, db’s not high enough to cause damage, but | find it hard to hear
instructions from the front of the room

| want this documented in case | need to escalate this to a safety incident

AP-

We reno’d the space (in the summer) and consult with a lot of parties, union, techs,
safety, trainers, regarding the server location

2 previous courses, with no concerns
The outcome of the test,
MH interjects- The range was between 56 and 63db
AP- Proves the levels were well within the acceptable range, did not reach 85db for 8 hours
MH interjects-, but were the fans high
BD- the fans are consistent

MH- no they’re not
